QUESTION 2: Critically discuss the implications of deinstitutionalisation and the movement towards Community Based Care for people with serious mental illness. Deinstitutionalisation and the development of community-based care (CBC) represent a paradigm shift in caring for the seriously mentally ill (SMI). It is no longer regarded as optimally therapeutic to render psychiatric care for the SMI in large institutions. Instead there is now a need to involve families and communities in such care. The shift towards deinstitutionalisation was the result of 3 factors - the discovery of psychotropic drugs (mood-altering drugs that lead to stabilisation of psychiatric patients), state budget cuts/ rationalisation of services, and recognition of the stigmatisation of institutionalisation of the SMI.
