H.S.E This paper examines accidents their cause, effect and prevention plus the accident investigation and reporting process. It is relevant to cross refer the paper to the papers on the Why, Who and How of H & S and Human Factors. Cause and Effect It would be far too simplistic to suggest that the sole cause of accidents is some form of human error by employees but it has already been established elsewhere that many accidents are attributed to human error.
